The On-Line Financial Disclosure System can only be used by individuals with an appointment at Washington University. Your Employee ID Number and corresponding password will be needed to access to this system. The Important Instructions provide specific information regarding your access to the system and step by step instructions to complete the form. You can also learn more about the system by reviewing the Frequently Asked Questions.

If you do not have an appointment at WU and need to complete a Financial Disclosure Statement, submit one of the following:
1) Complete this disclosure form
You will need Adobe Acrobat version 5.0 or higher to use this form. It is a fillable form; however, your signature is needed to complete the form. Once you have completed the form, you should mail it to the Disclosure Review Committee at 660 South Euclid Avenue, Campus Box 8093, St. Louis, MO 63110 or you can fax it to 314-362-8712.
2) You may submit the following letter signed by your institution:
