1. What is a Financial Disclosure Statement?
A Financial Disclosure Statement is the mechanism for an individual to disclose any personal financial interests that could be related to his or her research activities.

2. Why should I complete a Financial Disclosure Statement?
Washington University policies and Federal Regulations require these disclosures on an annual basis or as changes occur in your financial interests related to your research activities.

3. Who should fill out a financial disclosure statement?
All faculty should complete the disclosure statement. This includes full- and part-time faculty. In some instances this includes emeritus or adjunct faculty, if involved with research at the University. In addition, any individual involved in the design, conduct or reporting of research, regardless of appointment, should complete the disclosure statement.

4. When do I have to fill out the form?
You should complete the form annually or as changes occur with your financial interests related to your research activities. If you have previously completed a disclosure form, you will be reminded when your form is due for completion. If you are a new faculty member, you will be notified shortly after you are hired.

5. Where do I get the form?
You should complete this form on-line at http://researchgateway.wustl.edu, if you have a current appointment at the University. You should also read the Important Instructions carefully before completing and submitting the form.

6. What do I need to fill out the on-line form?
You must have your Employee ID Number and a password to submit this form on-line. Your Employee ID Number is the same as you would use to access the HRMS Self Service system or the Human Subjects Education module, or you can contact your business office or Systems and Procedures to obtain the number. If you have not already accessed HRMS Self Service or the Human Subjects Education module and created a password, you have a temporary password. Your temporary password is the last two digits of your birth year, the first character of your last name (lower case), and the last four digits of your SSN (in that order). For example, if your name is John Doe, you were born in 1960, and your SSN is 000-00-1234, your password would be 60d1234. Contact the Systems Help Desk at 935-5707 if you have problems accessing the system or to reset your password.

7. Who do I contact if I have a question?
